Refresh Renew Restore Wellness, PLLC provides individualized, evidence-based care, which may include:

• Preventative and wellness services 
• Hormone optimization (including bioidentical hormone therapy) 
• Metabolic and longevity-focused care 
• Laboratory testing and interpretation 
• Supplement and medication management 

All services are provided at the discretion of the provider after a paid appointment and based on clinical judgment and patient appropriateness.

The Practice operates on a transparent, cash-pay model.

• Initial Assessment: Payment is due at the time of service 
• Membership Fees: Billed monthly as agreed upon at enrollment 
• Laboratory Services: Due at the time labs are ordered or drawn 
• Additional Services: May incur additional fees disclosed in advance 

The Practice does not bill insurance. Patients are responsible for all fees associated with their care.

All payments are non-refundable unless otherwise specified in writing.

If enrolled in a membership:

• Membership provides access to ongoing care and services as outlined at enrollment 
• Fees are billed automatically on a recurring monthly basis 
• Cancellation requires written notice (as defined in your membership agreement) 
• Membership fees are non-refundable for partially used billing periods 

Failure to maintain an active membership may result in interruption of care services.

• Appointments must be scheduled in advance 
• Cancellations or rescheduling may require notice as defined by the Practice 
• Missed appointments may be subject to fees 

The Practice reserves the right to modify scheduling policies at any time.

All prescriptions, therapies, and recommendations are based on clinical evaluation and medical judgment.

• Individual responses to treatment vary 
• Patients are responsible for following treatment plans and reporting concerns 

Certain therapies, including hormone optimization and peptide therapies, may involve off-label use where appropriate and discussed during informed consent.

Laboratory testing is used to support clinical decision-making.

• Patients are responsible for all lab-related fees
